Health & Immunization Requirements

• MMR: Proof of two vaccines or positive IGG titer; no declinations accepted
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted but mask required
• Positive PPD or IGRA with negative chest X-ray within 1 year and annual TB questionnaire
• 2 Step PPD or IGRA within 30 days of start
• Varicella: Proof of immunity or two vaccines; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Fit Test: Required annually; preferred prior to start
• Health Statement or physical exam prior to first assignment, updated annually
• Ascension 12 Panel Drug Screen within 30 days prior to start, updated annually

Background & Verification

• Criminal Background Check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry (within 30 days of assignment)
• Employment Verification (third-party, past 7 years)
• Education Verification (within 90 days of start)
• Ascension 2 Professional References (at least two)
• Ascension Profile (resume or professional profile)
• Ascension EPLS (Exclusionary Party List System) check within 90 days prior to start
