  • Ascension Color Vision (updated every 3 years)
  • Ascension Core Competencies (HIPAA, Joint Commission, OSHA; updated yearly)
  • NIH Certification (Group A yearly, Group B-F every 2 years)
  • Ascension EKG Exam (passing score required)
  • MOAB Moderate Risk De-Escalation Training
  • Ascension Cerner Test-Out
• Driver’s License/State ID (must be valid and match address requirements)

Health & Immunization Requirements

• MMR: Proof of two vaccines or positive IGG titer; no declinations accepted
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
• Positive PPD or Negative Chest X-Ray and annual TB Questionnaire
• 2 Step PPD or IGRA (Quantiferon/Tspot) within required timeframes
• Varicella: Proof of immunity or two doses of vaccine; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Fit Test: Required annually; preferred prior to start
• Ascension 12 Panel Drug Screen: Must include all 12 drugs tested, with MRO review if applicable
• Health Statement or physical exam prior to first assignment, updated annually

Onboarding & Compliance Requirements

• Completion of Ascension Nursing Contract Worker Onboarding and related education attestations
• Ascension Skills Checklist (clinical skills verification) updated annually
• Criminal Background Check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry (updated annually)
• EPLS (Excluded Parties List System) check within 90 days of start
• Education Verification within 90 days of start
• Employment Verification for past 7 years
• Ascension Confirmation of Assignment documentation

Additional Notes

• Nursys and license verifications must be submitted prior to assignment start
• Some assessments and trainings require a minimum passing score (e.g., Ascension Core Competencies, EKG Exam) with limited attempts
• TB documentation and questionnaires must be updated annually
• Prior travel experience is NOT explicitly required for this position
