Position Details

• Ascension EKG Exam – Must pass with at least 80%, two attempts allowed
• MOAB Moderate Risk De-Escalation Training
• Ascension Cerner Test-Out – Completion required
• Driver’s License/State ID – Must be valid and match address requirements

Health and Immunization Requirements

• MMR – Proof of two vaccines or positive IGG titer; no declinations accepted
• Fit Test – Required annually; preferred prior to start
• Hepatitis B – Documentation of vaccination series completion or signed declination
• Influenza Vaccine – Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
• TB Screening – Positive IGRA or PPD results, negative chest X-ray within a year, and annual TB questionnaire
• Varicella – Proof of two vaccines, immunity, or history; no declinations accepted
• Tdap – Valid within 10 years; no declinations accepted
• 12 Panel Drug Screen – Within 30 days prior to start; includes 12 specified drugs; MRO review required for prescriptions

Onboarding and Compliance Requirements

• Health Statement – Statement of good health or physical exam prior to first assignment, updated annually
• Ascension Skills Checklist – Clinical skills checklist completed within one year of start, updated annually
• Ascension Profile and Professional References – Resume/profile and at least two professional references required prior to first assignment
• Background Checks – Criminal background check, EPLS, employment verification, education verification, and confirmation of assignment required; must be current and updated as specified
• Ascension Onboarding Requirements Education Attestation – Completion of pre-hire education modules
• Ascension Nursing Contract Worker Onboarding – Onsite completion required
• Ascension Nova StatStrip Training – Pre-hire modules required for non-allied positions
