Job Description

  • Position available at a medical/surgical unit.
  • Facility is part of a large hospital system.

Required Credentials and Onboarding Requirements

Licenses & Certifications

  • Nursys Verification: Must be completed within 30 days prior to start.
  • Current Nursing License: Primary source verification within 30 days prior to start.
  • BLS (AHA): Required before starting.
  • NIH Certification: Group A updated yearly; Groups B-F every 2 years.
  • MOAB Moderate Risk De-Escalation Training: Required.
  • Ascension EKG Exam: Must pass with at least 80%, max two attempts.
  • Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, max two attempts.
  • Ascension Cerner Test-Out: Must complete test and training checklist with signed attestation.

Health & Immunizations

  • MMR: Proof of two vaccines or positive IGG titer; no declinations accepted.
  • Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted.
  • Tdap: Valid within 10 years; no declinations accepted.
  • Hepatitis B: Documentation of vaccination series completion or signed declination.
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement.
  • 2 Step PPD or IGRA: Required with specific timing; annual TB questionnaire. Positive TB requires chest X-ray within 1 year.
  • Fit Test: Required for all travelers; preferred before start, can be done onsite.
  • Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
  • Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually. Includes 12 specific drugs; MRO review required for prescriptions or negative-dilute results.

Professional Requirements

  • Resume/Profile: Required for submission.
  • Two Professional References: Required prior to first assignment.
  • Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or after break in service >90 days.
  • Employment Verification: Past 7 years, updated annually or after break in service >90 days.
  • Education Verification: Relevant to clinical work, completed within 90 days of start; updated after break in service >90 days.
  • Driver’s License/State ID: Must be current, match address on file, and show front/back.
  • Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
  • Ascension Onboarding Education Attestation: Completion of pre-hire 6-hour module required.
  • Ascension Nursing Contract Worker Onboarding: Completed onsite.
  • Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
  • Ascension Skills Checklist: Clinical skills checklist completed within one year of start, updated annually.
  • Ascension Confirmation of Assignment: Provided upon initial assignment and updates with extensions.

Optional/Additional Information

  • Ascension Unit Specific Checklist: Optional for onboarding; uploaded by unit leader within a week.
  • Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended.
  • Hiring Manager Evaluation: Post-assignment evaluation (optional).
  • Ascension Name Change Document: Legal documentation of name change (optional).
