Job Description

• Every other weekend coverage
• Teamwork is essential
• Required certifications prior to starting: BLS, ACLS, NIH

Required Certifications & Training

• BLS (American Heart Association only)
• ACLS (American Heart Association only)
• 8 Hours of Stroke Training (includes NIH Stroke Scale training)
• Arnot Skills Checklist (unit-specific, updated annually)
• Arnot Core Competency Exam (must score 80% or higher)

Licenses & Verifications

• State License Verification (completed within 30 days prior to start)
• Driver's License or State ID (valid, unexpired US ID)

Health & Immunization Requirements

• COVID-19 Vaccine (optional: proof or declination)
• MMR (positive IgG titer or 2 vaccine series; must show individual results)
• Fit Test (completed within last 12 months; specific approved mask models)
• Hepatitis B (positive antibody titer, 3 vaccine series, or facility-specific declination)
• Influenza Vaccine (or facility-specific declination; mask required if declined)
• TB Test (annual requirement; specific testing protocols and follow-up for positives)
• Varicella (positive IgG titer or 2 vaccine series)
• Tdap (preferred within last 10 years; declination form if not available)
• Physical/Health Statement (within last 12 months; must include vitals, Snellen eye chart, and physician approval)

Background & Screening

• Agency Background Check (within 30 days of start; includes SSN trace, county and national criminal checks)
• Agency Drug Screen (within 30 days of start; excludes marijuana testing per NY state regulations)
• 7 Instant Background Checks (manual checks including NYOMIG, OFAC, OIG, and others)
• Internal Background Check (initiated upon acceptance; full disclosure required)

Documentation & Professional Requirements

• Arnot Resume (account for all employment and gaps for last 7 years; education history included)
• 2 Professional References (1 supervisor from last 12 months, 1 supervisor or peer from last 3 years)
• Employment Verification (most recent employer verification required)
• Job Description (signed and dated)
• Color Headshot (professional photo, no filters, JPEG format)
• Personal Information Form, Code of Conduct, Sexual Harassment Acknowledgement, Confirmation of Assignment Form, Self Study Packet and Quiz, Parking Registration Form (all required)
• AH Drug Tobacco Acknowledgement

Additional Notes

• Must be prepared for teamwork and flexible scheduling with every other weekend coverage.
• All required documentation and certifications must be completed and verified prior to start date.
• Fit testing can be done on-site if needed but must be communicated in advance.
