Background and Compliance Requirements

• Education verification: Degree from NLN or CCNE accredited program required

• Performance evaluations completed at end of assignment (not required prior)

Health and Immunization Requirements

• Physical exam within 1 year attesting ability to meet job demands
• Medical fit mask test within 1 year prior to start (N95 or PAPR required)
• Tetanus/Diphtheria/Tdap vaccine or declination
• Negative TB test (Q-gold/T-spot within 30 days of start or two PPDs including one within 30 days)
• TB questionnaire updated annually
• Measles (Rubeola), Mumps, Rubella, Varicella titers required; boosters if low titers
• Hepatitis B titer, vaccine series, or declination accepted
• Seasonal flu vaccine required (declinations not accepted except approved religious/medical exemptions)
• Drug testing within 30-60 days of start (9-panel minimum; instant drug screens not accepted)
• Latex questionnaire required prior to start

Competency and Orientation

• Medication/Pharmacology competency exam required for RNs
• Color blindness testing required
• Abuse reporting competency required (covering physical/sexual abuse, domestic violence, neglect/exploitation)
• Facility-specific exams at orientation including:

  • Pharmacology exam
  • Point of Care Testing
  • Airborne Precaution Mask Fitting
  • Basic Knowledge Assessment Tool (BKAT) for ICU, Tele/Step-Down, Progressive Care units
• Orientation includes completion of assigned learning modules (~6 hours)
