Job Opportunity: RN Telemetry I

Location: Elmira, New York
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Required Certifications & Licenses

  • BLS (American Heart Association only)
  • ACLS (American Heart Association only)
  • State Nursing License: Verification required within 30 days prior to start
  • Driver's License or State ID: Valid, unexpired US license or ID

Required Health & Immunization Documentation

  • COVID-19 Vaccine: Proof or agency declination (optional)
  • MMR: Positive IgG titer or 2 vaccine series required; titers must show results for Measles, Mumps, and Rubella individually
  • Varicella: Positive IgG titer or 2 vaccine series required
  • Hepatitis B: Positive antibody titer, 3 vaccine series, or facility-specific declination form required
  • Influenza Vaccine: Required or facility-specific declination form (mask required if declined)
  • Tdap: Preferred but not required; if no record, declination form needed
  • TB Test: Must have one of the following within required timeframes:
    • 2 skin PPDs (1 within 12 months, 1 within 3 months) or
    • 1 Quantiferon/Spot test within 12 months
    • For past positives, chest X-ray within 12 months may be required
  • Fit Test: Must be completed prior to start and within last 12 months; specific approved mask models required
  • Physical/Health Statement: Within last 12 months, including physician credentials, vitals, and statement of no restrictions

Professional Requirements

  • Skills Checklist: Completed for the specific unit, updated annually
  • Core Competency Exam: Must score 80% or higher on Core Comp I, II, and III for RNs
  • Resume: Complete employment and education history for last 7 years, with dates and locations
  • Professional References:
    • 1 supervisor reference from last 12 months
    • 1 supervisor or peer reference from last 3 years
  • Employment Verification: Verification of most recent employer (agency verification accepted)
  • Background Check: Must be run within 30 days of start, including SSN address trace, county criminal searches for all counties lived/worked in last 7 years, FACIS 3 check, and national criminal check
  • Drug Screen: Must be run within 30 days of start; testing excludes marijuana per New York state regulations
  • Color Headshot: Recent, clear photo with solid background, no filters or sunglasses
