Job Opportunity: Emergency Department (ED) Case Manager - Weekend
Location: Fairfield, California
Employment Type: Clinical Nursing Travel
Contract Length: 5 weeks
Job Summary
The ED Case Manager Weekend is responsible for assessing, planning, implementing, monitoring, and evaluating individualized care plans for patients in the Emergency Department and across the continuum of care. The role ensures timely patient transitions to the optimal level of care, collaborates with interdisciplinary teams, and coordinates post-ED care. The Case Manager uses clinical knowledge, communication, problem-solving, and organizational skills to drive optimal patient outcomes while considering financial and health plan resources.
Key Responsibilities
- Apply MCG criteria to monitor appropriateness of admissions, level of care, and continued stays.
- Document findings and interventions in Cerner according to departmental standards.
- Manage patient care through the ED and healthcare systems based on individual needs.
- Collaborate with physicians, Social Services, and healthcare providers for care plan changes.
- Advocate for patients and families throughout the care episode.
- Develop individualized comprehensive care plans with the interdisciplinary team.
- Continuously assess and update discharge plans as needed.
- Transition patients to the next level of care and involve appropriate outpatient resources.
- Coordinate patient transfers using health plan knowledge and MCG criteria.
- Initiate and facilitate outpatient referrals to prevent inappropriate hospital admissions.
- Maintain knowledge of federal, state, and local regulations, including Medicare rules and Conditions of Participation.
- Act as a resource for physicians and staff on utilization management and care options.
- Communicate with health plans and act as a liaison regarding insurance and discharge planning.
- Identify high-risk patients and communicate with Readmissions Case Manager.
- Evaluate case management impact on outcomes and develop strategies for improvement.
- Maintain confidentiality of patient and electronic data per regulations.
- Keep current on organizational policies, regulations, and legislative changes.
Required Skills & Experience
- Minimum of 3 years acute clinical nursing experience (highest required years of experience).
- Minimum of 2 years case management experience.
- Basic computer skills including word processing and data entry.
- Competence in Microsoft Office programs (Outlook, Word).
- Knowledge of Medicare rules, Joint Commission, Department of Health standards.
- Understanding of acute care, home care, subacute care, long-term care, hospice, rehabilitation, and community resources.
- Familiarity with medical necessity guidelines, quality assurance processes, and healthcare trends.
Preferred Skills & Experience
- Experience with MCG criteria.
- Experience with Cerner electronic health records.
- Experience with PowerPoint.
- Emergency Department experience.
Education Requirements
- Required: Graduate of an accredited nursing school with a Bachelor of Science in Nursing (BSN).
- Preferred: Master of Science in Nursing (MSN).
Certifications & Licensure
- Required: Current California Registered Nurse (RN) license.
- Preferred: Certified Case Manager (CCM) or Accredited Case Manager (ACM).
Prior Travel Experience
- Not explicitly required.
