Job Description

- License/Certificate Verification within 30 days prior to start
- Driver’s License/State ID (must be current and valid)
Required Exams & Competency Assessments
- Ascension Specialty Exam (minimum 80% passing score, 2 attempts allowed)
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% passing score)
- Ascension EKG Exam (minimum 80% passing score, 2 attempts allowed)
- Ascension Skills Checklist (clinical skills competency, updated annually)
- Ascension 12 Panel Drug Screen (within 30 days prior to start, includes 12 specified drugs)
Health & Immunization Requirements
- MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
- Hepatitis B: Vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
- Varicella: Proof of immunity or two doses of vaccine (no declinations accepted)
- Tdap: Valid within 10 years (no declinations accepted)
- TB Screening: Positive IGRA or PPD results, negative chest X-ray within 1 year, and annual TB questionnaire
- Health Statement or physical exam prior to assignment, updated annually

- Ascension Cerner Test-Out (training and test checklist)
- Ascension Color Vision Test (updated every 3 years)
- Ascension Onboarding Requirements Education Attestation
- Ascension Nursing Contract Worker Onboarding (onsite)
- Ascension Nova StatStrip (pre-hire modules)
- Ascension Criminal Background Check (including Social Security Trace, OIG Search, National Sex Offender)
- Ascension EPLS (Exclusionary Party List System) check within 90 days of start
- Ascension Education Verification (within 90 days of start)
- Ascension Employment Verification (past 7 years)
- Ascension Confirmation of Assignment (agency letterhead documentation)
- Ascension Badge Photo (professional headshot with white or blue background)
