    Job Description

    • Must have Home Health Case Manager experience.
    • Responsible for overall patient management including direct care with designated visit productivity standards.
    • Plan, organize, and direct all patient care services for assigned caseload in compliance with current standards and regulations.
    • Coordinate interdisciplinary team to implement patient care plans.
    • Manage visit utilization and control expenses.
    • May assist with agency-wide orientation, competency evaluations, and provide clinical consultation related to expertise.

    Required Certifications

    • California RN License
    • BLS (Basic Life Support)
    • ACLS (Advanced Cardiac Life Support) - AHA or ARC only
    • PALS (Pediatric Advanced Life Support) - AHA or ARC only (verify by position)
    • NRP (Neonatal Resuscitation Procedure) - AHA/AAP only, must be Instructor or Advanced Provider Course (verify by position)
    • CCRN (Critical Care RN) (verify by position)
    • TNCC (Trauma Nursing Core Course) - valid for 4 years (verify by position)
    • EFM (Electronic Fetal Monitoring) - typically for L&D (verify by position)
    • State License Verification (primary source online verification within 30 days of start)
    • NURSYS Verification (within 30 days of start)

      • Seasonal Flu Vaccination during flu season (Nov-April) or signed declination
    • Drug Testing: Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids due to CA law).

    Competency Exams

    • Medication Test/Pharmacology Competency Exam as applicable.
    • Competency Exam with minimum passing score of 80% or Angoff score if higher.
    • Dysrhythmia/EKG Competency Exam may be required for some units.
