  • Physical/Health Statement: Must be from within last 12 months, including vitals and vision screening, with no restrictions.

Additional Professional Requirements

  • Skills Checklist: Must complete unit-specific skills checklist annually.
  • Core Competency Exam: Must score 80% or higher on nursing core competency exams.
  • Driver’s License/State ID: Valid, unexpired US driver’s license or state ID required.
