• Testing & Exams:

  • Prime Basic Pharmacology Exam (must score 80% or higher)
  • Prime Basic Rhythm Interpretation Screening (must score 80% or higher)
  • Prime Core Competencies Exam (must score 80% or higher)
  • Prime Specialty Exam (must score 80% or higher)
  • Prime Skills Checklist (does not expire on assignment)
  • Prime Color-blind Test (annual)

• Health & Background:

  • Annual Respirator Fit Test (must be completed prior to start if not available onsite)
  • Physical/Health Statement (annual, must state clear to work)
  • National Criminal Background Check (within one year of start date)
  • Prime Drug Screening: Minimum 10-panel test including THC saliva test (within 60 days of start)

• Other Requirements:

  • Prime CA Sexual Harassment Prevention Training (valid for 2 years)
  • Prime OIG, Prime SAM/EPLS, Prime Criminal Background Check, Prime Agency Attestation
  • Prime Education Verification (diploma or transcripts accepted if third-party verification unavailable)

  • Prime Form I-9 (Employment eligibility verification)

Additional Notes

• Flu and Hepatitis B vaccine declinations are not accepted at this facility.
• All credentials must be uploaded and verified prior to onboarding.
