Job Description

• Work areas include Medical/Surgical, Telemetry, and Stroke units.

Required Certifications & Training

• BLS (Basic Life Support) – AHA (American Heart Association) only
• ACLS (Advanced Cardiovascular Life Support) – AHA only
• 8 Hours of Stroke Training (includes NHISS training)
• State License Verification (completed within 30 days prior to start)
• Arnot Skills Checklist (unit-specific, updated annually)
• Arnot Core Competency Exam (must score 80% or higher)
• Fit Test (within last 12 months; specific mask models required)
• Agency Background Check (within 30 days of start)
• Agency Drug Screen (within 30 days of start; specific substances tested; marijuana testing prohibited by NY state)

Required Immunizations & Health Requirements

• MMR (Measles, Mumps, Rubella): Positive IgG titer or 2 vaccine series required
• Hepatitis B: Positive antibody titer, 3 vaccine series, or facility-specific declination form required
• Influenza Vaccine: Required annually or facility-specific declination form (mask required if declined)
• TB Test: Must have one of the following within last 12 months: 2 PPD skin tests or 1 Quantiferon/Spot test; CXR may be required for past positives
• Varicella: Positive IgG titer or 2 vaccine series required
• Tdap: Preferred within last 10 years; if no record, facility-specific declination form required
• Physical/Health Statement: Completed within last 12 months, including vitals and vision test, with no restrictions
• COVID-19 Vaccine: Upload proof or agency declination (optional)

Professional Requirements

• Valid, unexpired US Driver’s License or State ID
• Current color photo (headshot)
• Complete employment history for last 7 years (including gaps)
• Education verification (ASN or BSN for RNs)
• Two professional references (one within last 12 months, one within last 3 years)
• Signed and dated job description acknowledgment
• Employment verification (agency verifications count)
• Completion of facility-specific forms and acknowledgments (e.g., Code of Conduct, Sexual Harassment Acknowledgement)
• Completion of facility-specific self-study packet and quiz
• Completion of 7 instant background checks (including NYOMIG, OFAC, OIG, SAM, and others)
