Job Description

• Position based at St Francis Campus (medical/surgical unit)
• Travel assignment

Required Credentials & Certifications

• BLS (Basic Life Support) - Must be AHA certified
• Nursys Verification - Within 30 days prior to start; disciplinary actions require FACIS Questionnaire
• Current Nursing License - Primary source verification within 30 days prior to start
• Ascension Color Vision Test - Completed prior to start and updated every 3 years
• Ascension Core Competencies - HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly; minimum 80% score
• NIH Certification - Group A updated yearly, Groups B-F every 2 years
• Ascension EKG Exam - Must pass with at least 80%, two attempts allowed; updated as new editions released
• MOAB Moderate Risk De-Escalation Training
• Ascension Cerner Test-Out - Cerner system training and test results required
• Ascension Skills Checklist - Clinical skills checklist completed within one year of start; updated annually

Health & Immunization Requirements

• MMR - Proof of two vaccines or positive IGG titer; no declinations accepted
• Fit Test - Required annually; preferred prior to start
• Hepatitis B - Vaccination series completion or signed declination
• Influenza Vaccine - Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
• Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire - Must provide positive IGRA or PPD results, chest x-ray within a year, and completed TB questionnaire
• 2 Step PPD OR IGRA (Quantiferon/Tspot) - Within 30 days prior to start
• Varicella - Proof of two vaccines, immunity, or history; no declinations accepted
• Tdap - Valid within 10 years; no declinations accepted
• 12 Panel Drug Screen - Within 30 days prior to start; includes amphetamines, cocaine, marijuana, PCP, methamphetamine, barbiturates, benzodiazepines, buprenorphine, methadone, oxycodone, morphine, ecstasy; must include MRO review if applicable
• Health Statement - Statement of good health or physical exam prior to first assignment; updated annually

Professional Requirements

• Driver's License/State ID - Must be current and valid; address must match home state for compact licenses
• Resume/Profile - Required for submission
• Two Professional References - Qualifying references completed prior to first assignment
• Criminal Background Check - Includes Social Security Trace, OIG Search, National Sex Offender; completed within 30 days of first assignment; updated annually or after break in service > 90 days
• EPLS (Excluded Parties List System) Check - Within 90 days of start; rerun if break in service > 90 days
• Education Verification - Must be completed within 90 days of start; official transcripts required if verification fails
• Employment Verification - Third-party verification for past 7 years; updated annually or after break in service > 90 days
• Ascension Confirmation of Assignment - Provided upon initial assignment and updates with extensions
• Ascension Badge Photo - Forward-facing headshot with white or blue background; no alterations or filters
