Job Description
Seeking a Registered Nurse (RN) specialized in Medical/Surgical for a travel assignment. The position requires working within a hospital setting providing patient care on medical and surgical units.
Required Credentials & Certifications
- BLS (Basic Life Support) - Must be AHA certified.
- Nursys Verification - Within 30 days prior to start.
- Current Nursing License - Primary source verification within 30 days prior to start.
- Ascension Color Vision Test - Completed prior to start and updated every 3 years.
- Ascension Core Competencies - Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly (minimum 80% score, max 2 attempts).
- NIH Certification - Group A updated yearly; Groups B-F every 2 years.
- Ascension EKG Exam - Must pass with at least 80%, max 2 attempts.
- Ascension Onboarding Requirements Education Attestation - Completion required.
- Ascension Nursing Contract Worker Onboarding - Completed onsite.
- Ascension Nova StatStrip - Pre-hire modules required.
- MOAB Moderate Risk De-Escalation Training - Required.
- Ascension Cerner Test-Out - Required.
- MMR Vaccination - Documented proof of two vaccines or positive IGG titer; no declinations accepted.
- Fit Test - Required annually; preferred prior to start.
- Hepatitis B Vaccination - Documentation of series completion or signed declination.
- Influenza Vaccine - Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement.
- Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire - Must provide positive IGRA or PPD results, chest x-ray within a year, and completed TB questionnaire.
- Health Statement - Statement of good health or physical exam prior to first assignment, updated annually.
- 2 Step PPD OR IGRA (Quantiferon/Tspot) - Required within 30 days prior to start.
- Varicella Vaccination - Proof of two vaccines, immunity, or history; no declinations accepted.
- Tdap Vaccination - Valid for 10 years; no declinations accepted.
- Ascension 12 Panel Drug Screen - Within 30 days prior to start; includes testing for 12 specified drugs.
- Driver's License/State ID - Must be current and valid; address must match home state for compact licenses.

- Two Professional References - Required prior to first assignment.
- Criminal Background Check - Includes Social Security Trace, OIG Search, National Sex Offender; updated annually or after break in service > 90 days.
- EPLS (Excluded Parties List System) Check - Within 90 days prior to start; rerun if break in service > 90 days.
- Ascension Skills Checklist - Clinical skills checklist completed within one year of start; updated annually.
- Education Verification - Must be completed within 90 days prior to start; rerun if break in service > 90 days.
- Confirmation of Assignment - Provided upon initial assignment and updates with extensions.
- Employment Verification - Third-party verification for past 7 years; updated annually or after break in service > 90 days.
