Job Description

• Must have ACLS, NIH, and BLS certifications.
• Responsibilities include floating to other units and occasional charge nurse duties.

Required Certifications & Training

• BLS (Basic Life Support) – AHA (American Heart Association) only
• ACLS (Advanced Cardiovascular Life Support) – AHA only
• 8 Hours of Stroke Training (including NIH Stroke Scale)
• State License Verification (completed within 30 days prior to start)
• Arnot Skills Checklist (unit-specific, updated annually)
• Arnot Core Competency Exam (must score 80% or higher)
• Fit Test (within last 12 months; specific mask models required)
• MMR (Measles, Mumps, Rubella) – Positive IgG titer or 2 vaccine series required
• Hepatitis B – Positive antibody titer, 3 vaccine series, or specific declination form
• Influenza Vaccine – Required or specific declination form (mask required if declined)
• TB Test – Valid testing within last 12 months; follow-up for past positives may be required
• Varicella – Positive IgG titer or 2 vaccine series required
• Tdap – Preferred within last 10 years or specific declination form
• Physical/Health Statement – Completed within last 12 months, including vitals and vision test

Professional Requirements

• Valid, current US driver’s license or state ID (not expired)
• Agency background check (within 30 days of start) including criminal and county checks for all counties lived/worked in last 7 years
• Agency drug screen (9+ panel, within 30 days of start)
• Resume detailing all employment and education history for last 7 years
• Two professional references (one within last 12 months, one within last 3 years)
• Signed and dated job description acknowledgment
• Employment verification (agency verifications count; consecutive assignments under same agency count as one employer)
• Education verification (ASN or BSN for RNs)
• Color headshot photo (professional, no filters)
• Completion of various facility-specific forms and acknowledgments (e.g., code of conduct, sexual harassment acknowledgment, parking registration)

Summary
