Description
Job Opportunity: Med/Surg/Telemetry RN (Telemetry I)
Location: Hartford, Connecticut
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Description
- Work schedule: 36 hours per week every other week (EOW), with consideration for 48 hours per week
- Telemetry experience required
- Certifications required: BLS, ACLS
- NIHSS certification preferred
Required Experience
- Specialty: RN Telemetry I
- Years of experience: Not explicitly stated, but telemetry experience is required (assume minimum 1+ year telemetry experience)
- Prior travel experience: Not required
Required Certifications & Credentials (Must be current and verifiable)
- BLS (Basic Life Support)
- ACLS (Advanced Cardiovascular Life Support)
- Valid RN license with verification dated within 30 days of start
- Fit Test
- COVID-19 Vaccine (including booster or approved declination/exemption)
- MMR (Measles, Mumps, Rubella) vaccination or immunity proof
- Hepatitis B vaccination series (titers not accepted)
- Influenza Vaccine (or declination/exemption) for current flu season
- 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per state requirements
- Varicella vaccination or immunity proof
- Physical/Health Statement confirming fitness to work (within last year)
- Trinity 10 Panel Drug Screen (completed within 30 days of start)

- Trinity Code of Conduct Form
- Trinity I9 Form
- Confidentiality and System Security Agreement
- Integrity and Audit Services Form
- Specialty Specific Competency Testing (completed within last year)
- Skills Checklist (provided by agency for clinical contractors)
- Criminal Background Check (federal, nationwide, county for last 7 years)
- Education Verification (highest academic degree or diploma/GED)
- EPLS (Excluded Parties List System) check
- GSA/OIG (Office of Inspector General) check
- Joint Commission CORE Exam (within last year)
- NSO (National Sex Offender) check
- System Access Form (completed and uploaded one week before start)
- Conflict of Interest Disclosure Form
- OFAC (Office of Foreign Assets Control) check
- Trinity Profile (resume, skills checklist, 2 professional references)
Additional Notes
- All certifications and verifications must be attached/scanned and uploaded as required
- Drug screen panel excludes THC
- Some forms require signatures within days prior to start
- Declinations for vaccines accepted only where specified and with agency letterhead
- Specialty competency exam and skills checklist must be current (within 1 year)
