Job Opportunity: RN Case Manager (Days)

Location: Northampton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Job Description

  • Work schedule: 8- or 10-hour shifts, 40 hours per week, typically 8:00 AM to 4:30 PM
  • Work every other weekend and every other holiday
  • Responsible for providing direction and leadership for patient progression and care coordination
  • Collaborate with internal and external disciplines to direct and coordinate patient care from admission to transfer or discharge
  • Focus on cost-effective utilization of hospital resources, minimizing patient out-of-pocket expenses, appropriate resource utilization, and patient advocacy
  • Operates within established organizational and departmental policies and procedures

Required Qualifications

  • Graduate of an approved, accredited school of professional nursing (BSN preferred)
  • Licensure as a Registered Nurse (RN) in the Commonwealth of Massachusetts required
  • Case Management certification preferred or active pursuit of certification
  • Experience: 1 to 2 years of related clinical, continuing care, or case management experience required (use higher value: 2 years)
  • Excellent written and oral communication skills
  • Experience with EPIC electronic health record system required

  • MGB Resume including past 7 years of work experience (with city & state)
  • 2 recent references, at least 1 supervisor or above in skillset

  • MGB Candidate Submission Coversheet
  • MGB Badge Photo (color photo, recent within 6 months, plain background)
  • Documentation of immunizations and health screenings including:
    • MMR (measles, mumps, rubella) vaccination or proof of immunity
    • Influenza vaccine during flu season (Sept 15 - April 30) with no exemptions
    • TB test within 3 months of start date or negative QuantiFERON/T-spot blood test
    • Health statement signed by MD/DO/NP/PA within 365 days of start date
  • Criminal background checks including:
    • MGB Criminal Background Check (within 45 days prior to start)
    • Massachusetts CORI background check (within 30 days prior to start)
    • National Sex Offender Check (within 30 days prior to start)
    • Exclusion checks (OIG, GSA, MassHealth & NH Medicaid) within 30 days prior to start and annually
  • Education verification through background check company
  • Respirator Medical Clearance Attestation if fit testing is required
  • Confirmation of Assignment/POI Form upon acceptance and for extensions
