Description
Job Opportunity: CT Technologist
Location: Maywood, Illinois
Employment Type: Long-Term Assignment (LTA)
Shift: 3:00 PM - 11:30 PM, including every third weekend
Contract Length: 12 weeks
Required Qualifications
- Valid CT Technologist license in Illinois
- ARRT - CT certification required
- Minimum of 1 year of CT Technologist experience within the last year
- American Heart Association BLS (Basic Life Support) certification
- Fit Test
Required Onboarding and Compliance Documents
- BLS certification (front and back images)
- License/Certificate verification dated within 30 days of start
- Fit Test
- 10 Panel Drug Screen completed within 30 days of start (no THC)
- Signed Trinity Code of Conduct Form
- Standard I9 form
- Confidentiality and Information System Security Agreement
- Integrity and Audit Services Form
- Motor Vehicle Report (MVR) dated within 30 days of start
- COVID-19 Vaccine proof (booster required, no declination accepted in IL)
- MMR vaccination proof or IgG titer
- Hepatitis B vaccination series proof (titers not accepted)
- Influenza vaccine proof (no declination accepted in IL)
- 2 Step PPD OR IGRA (Quantiferon/Tspot) test
- Varicella vaccination proof or IgG titer
- Physical/Health Statement dated within last year
- Resume, Skills Checklist (within last year), and 2 Professional References
- Conflict of Interest Disclosure Form
- Criminal Background Check (Federal, Nationwide, County - last 7 years) with SSN verification dated within 30 days
- Education Verification (highest level of academic education)
- EPLS (Excluded Parties List System) report dated within 30 days
- GSA/OIG Exclusion report dated within 30 days
- Joint Commission Testing (CORE Exam within last year)
- NSO (National Sex Offender) check dated within 30 days
- System Access Form (to be completed and uploaded one week before start)
- Skills Checklist (provided by agency for clinical contractors)
Additional Information
- Must be able to work every third weekend
- All documentation must be uploaded to the agency’s system prior to start date as specified
- Compliance with all vaccination and health screening requirements is mandatory
This position is ideal for a CT Technologist with ARRT-CT certification seeking a 12-week long-term assignment in Illinois with evening shifts and weekend rotation.
