Job Opportunity: RN - Case Management (Travel)
Location: San Leandro, California

Job Description
- Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
- Provide comprehensive assessment, care planning, plan implementation, and evaluation of individual patient needs.
- Enhance quality of patient management and satisfaction, promoting continuity of care and cost effectiveness.
- Integrate case management, utilization review, and discharge planning functions.
- Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
- Collaborate strongly with interdisciplinary teams (IDT).
Schedule
- Monday to Friday, 8:00 AM – 4:30 PM
Contract Length
- 13 weeks
Required Qualifications
- License: California RN license required.
- Certifications: BLS certification through the American Heart Association required.
- Experience: Minimum 2 years recent RN Case Management experience required.
- Experience must include LTAC or STACH settings, MCG guidelines, and discharge planning across acute care settings.
- Strong interdisciplinary team collaboration skills required.
