Job Opportunity: RN - Case Management (Travel)

Location: San Leandro, California
Employment Type: Travel

Job Description

Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
Provide comprehensive assessment, care planning, plan implementation, and evaluation of individual patient needs.
Enhance quality of patient management and satisfaction, promoting continuity of care and cost effectiveness.
Integrate case management, utilization review, and discharge planning functions.
Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
Collaborate strongly with interdisciplinary teams (IDT).

Schedule

Monday to Friday, 8:00 AM – 4:30 PM

Contract Length

13 weeks

Required Qualifications

Valid RN license in California
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Minimum 2 years of Case Management experience, including within the last year
Experience in Acute Care and Long Term Acute Care (LTAC) settings
Experience with MCG guidelines
Proficient in discharge planning
BLS certification issued by the American Heart Association
Demonstrated RN Case Management experience
