Job Opportunity: RN - Homecare

Job Summary

  • Provide professional nursing care to adult and pediatric Hospice/Home Health patients and their families in a home setting.
  • Responsible for assessing, planning, implementing, and evaluating patient care.
  • Work autonomously while delivering care in compliance with hospice/home health policies, legal, and regulatory standards.
  • Utilize nursing process, technology, therapeutic interventions, evidence-based practice, and coordinate care with interdisciplinary team members.
  • Perform assessments of physical, social, psychological, cognitive, communicative, and developmental status.
  • Collaborate with patients, families, physicians, and other team members to develop and update individualized care plans.
  • Direct and coordinate care delivery through delegation and collaboration.
  • Evaluate patient responses and revise care plans as needed.
  • Maintain professionalism by adhering to legal and ethical standards, providing accurate documentation, and promoting nursing development.
  • Electronically transfer patient files to the office at least twice daily.

Contract Length

  • Approximately 13 weeks (89 days rounded to nearest week).

Required Qualifications

  • Valid Registered Nurse (RN) license in Utah.
  • Minimum 2 years of Home Health nursing experience, including experience within the last year.
  • Experience in Acute Care and Hospice settings.
  • Basic Life Support (BLS) certification.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Experience working with adult and pediatric patients.
  • Ability to communicate effectively both verbally and in writing.
  • Basic computer skills.
  • Ability to transfer patients independently.
  • Reliable access to the internet or ability to commute to the office twice daily for electronic file transfers.

Physical Requirements

  • Ability to see and read information, labels, and assess patient needs.
  • Frequent verbal communication and ability to hear alarms and spoken information quickly and accurately.
  • Manual dexterity for handling complex equipment and frequent computer use.
  • Ability to lift and transfer patients, bend, carry supplies and equipment, including heavy items.
  • Ability to walk, assist with patient ambulation, and navigate crowded or uneven environments.
  • Ability to drive a vehicle safely.

Preferred Qualifications

  • Minimum 2 years of experience as an RN.
  • Previous experience in Acute Care, Home Health, or Hospice settings.

Generally speaking, Host Healthcare doesn't always have the highest rate on the market. Having gone with competing agencies in the past, I have often been disappointed and unsupported and ultimately found it wasn't worth the small pay bump
