Job Description

• Every Other Week (EOW) schedule
• Specialty: RN Medical / Surgical / Telemetry

Required Qualifications

• License: Current and valid RN license for Iowa
• Certifications:

  • BLS (Basic Life Support) - American Heart Association certification required for all direct patient care personnel
  • ACLS (Advanced Cardiovascular Life Support) - required for RNs in critical care areas
  • NIHSS (National Institutes of Health Stroke Scale) certification
• Drug Screening:

  • 10-panel drug screen completed within 30 days of start (includes cocaine, amphetamines, opiates, PCP, barbiturates, benzodiazepines, methaqualone, methadone, and others; NO THC)
• Health Requirements:

  • COVID-19 vaccine (booster by Sept 2025) or approved exemption/declination (declination not accepted for CA or IL)
  • MMR vaccination or immunity proof
  • Hepatitis B vaccination series (proof of vaccination required; titers not accepted)
  • Influenza vaccine or declination for the current flu season
  • Varicella vaccination or immunity proof
  • Tdap vaccination (declinations accepted if no pediatric interaction)
  • 2 Step PPD or IGRA (Quantiferon/Tspot) for TB screening
  • Physical/Health Statement confirming fitness to work (exam within last year)
• Background Checks and Compliance:

  • Federal, nationwide, and county criminal background check (last 7 years) including SSN verification, dated within 30 days of start
  • Iowa-specific criminal history, sex offender, dependent adult & child abuse registries check (within 30 days prior to start)
  • EPLS (Excluded Parties List System) check within 30 days
  • OIG/GSA exclusion check within 30 days
  • NSO (National Sex Offender) check within 30 days
  • OFAC (Office of Foreign Assets Control) sanctions check within 30 days
• Additional Requirements:

  • Fit Test (respirator mask fit testing)
  • Specialty-specific competency testing completed within the last year
  • Trinity Health Code of Conduct, Confidentiality Agreement, Integrity and Audit Services Form, Conflict of Interest Disclosure, and other professional compliance forms signed and on file
  • Trinity Skills Checklist and Profile (resume, skills checklist within a year, 2 professional references)
  • License/Certificate verification dated within 30 days of start (state verification such as NURSYS for RNs)
  • Standard I9 verification completed by agency
  • Trinity System Access Form completed and uploaded one week before start

Summary
