Job Opportunity: Case Manager (Registered Nurse)
Location: San Francisco, California
Employment Type: Travel/Contract
Contract Length: 15 weeks
Job Description
- Responsible for Care Coordination and Care Transitions Planning throughout the acute care patient experience.
- Collaborates with physicians, utilization managers, medical social workers, bedside RNs, and interdisciplinary teams to ensure timely patient progression and appropriate level of care.
- Aims to prevent unnecessary admissions or readmissions by facilitating communication and resource coordination across the care continuum.
- Works with patients, families, nursing management, quality and ancillary services, third-party payers, review agencies, claims and finance departments, medical directors, contracted providers, and community resources.
- If assigned to the Emergency Department, addresses complex clinical and social situations efficiently to avoid unnecessary admissions.
Shift: 4 shifts of 10 hours (0800-1830), rotating weekends, no on-call required.
Required Qualifications
- Valid Registered Nurse (RN) license in California.
- Minimum 2 years of case management experience, including experience within the last year.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Experience in utilization management, including working with attending physicians to support level of care protocols.
Certifications (as applicable)
- ACLS (Advanced Cardiac Life Support) – AHA or ARC only.
- BLS (Basic Life Support) – AHA or ARC only.
- PALS (Pediatric Advanced Life Support) – AHA or ARC only (verify by position).
- NRP (Neonatal Resuscitation Program) – AHA/AAP only, must be Instructor or Advanced Provider Course (verify by position).
- CCRN (Critical Care RN) – verify by position.
- TNCC (Trauma Nursing Core Course) – valid for 4 years (verify by position).
- EFM (Electronic Fetal Monitoring) – typically required for Labor & Delivery (verify by position).
- CPI (Crisis Prevention Institute) – verify by position.
Background and Screening Requirements
- 7-year background check required within 6 months of start, including social security trace with all aliases, county searches, nationwide sex offender search, education verification, and motor vehicle report if applicable.
- FACIS Level 3 screening required within 2 weeks of start and monthly thereafter (or equivalent checks if FACIS III cannot be run).
- EPLS/GSA/SAM and HHS/OIG checks required and updated annually.
- Sex Offender Search required within 90 days of start.
- Education verification required (not required if highest education is high school). Bachelor of Science in Nursing preferred.
- Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law).
- Physical exam within 1 year of start, including clearance for work.
- Fit mask test within 1 year of start, renewed annually (exceptions apply).
- TB screening questionnaire within 90 days of start, renewed annually, with negative TB test or chest x-ray as required.
- Immunizations and titers required: Tetanus/Diphtheria/Tdap, Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination during flu season. Medical or religious exemptions considered with proper documentation.
- Respiratory safety clearance required prior to start and renewed annually (exceptions apply).
Documentation and Compliance
- Must provide full 7 years of work history with no gaps, including city and state of employment.
- Employment verification required for up to 7 years; gaps over 30 days must be explained. Attestations accepted only under specific conditions.
- References: 2 professional references required for clinical positions within 3 years of hire.
- Must review and sign facility-specific documents including job description, standards of business conduct, code of conduct, vendor training, overpayment refund policy, and regulatory orientation prior to start.
- Additional facility-specific forms may be required for religious exemptions or other accommodations.
