Job Opportunity: Case Manager Registered Nurse
Location: Oakland, California
Employment Type: Travel/Contract
Contract Length: 26 weeks
Job Summary
Seeking an experienced Case Manager Registered Nurse (RN) to coordinate care and manage patient transitions throughout the acute care experience. The role involves collaboration with physicians, utilization managers, social workers, and nursing staff to ensure timely and appropriate patient care transitions, prevent unnecessary admissions/readmissions, and optimize resource utilization.
Work Schedule
- 5/8 shifts
- Every other weekend
Key Responsibilities
- Conduct initial and ongoing patient assessments, including medical, psychosocial, and financial information.
- Lead healthcare teams in developing cost-effective care coordination plans.
- Oversee case management assistants in post-acute care service planning.
- Perform utilization management by reviewing medical records to confirm level of care requirements.
- Collaborate with physicians and interdisciplinary teams to expedite patient transitions.
- Monitor length of stay and identify outliers requiring additional focus.
- Ensure compliance with Medicare and regulatory requirements related to care transitions.
- Develop and communicate transition plans involving patients, families, payers, and community resources.
- Identify barriers to care progression and work with multidisciplinary teams to resolve them.
- Address complex clinical and social situations to avoid unnecessary admissions and delays.
- Participate actively in department operations and continuous improvement initiatives.
- Maintain professional relationships with healthcare teams and community agencies.
- Communicate effectively and provide/accept constructive feedback.
Required Qualifications
- Valid RN license in California
- Minimum 5 years of Case Management experience, including experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Advanced Cardiovascular Life Support (ACLS) certification issued by American Heart Association or American Red Cross
- Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
- National Tech Certification Verification
Background and Compliance Requirements
- 7-year background check including social security trace, county searches, sex offender search, and education verification.
- Drug screening (9-panel, excluding marijuana/cannabinoids due to California law).
- Immunizations and health screenings including:
- TB test or chest x-ray (annual renewal)
- Tetanus/Diphtheria/Tdap (within 10 years)
- Measles, Mumps, Rubella (MMR) immunity or vaccination
- Hepatitis B vaccination or declination
- Varicella immunity or vaccination
- Seasonal flu vaccination during flu season
- Physical exam within 1 year of start.
- Fit mask test within 1 year, renewed annually.
- Respiratory safety clearance (not required for all roles).
- References and employment history verification covering 7 years.
- Completion of facility-specific documents and orientation prior to start.
Additional Notes
- Must comply with all regulatory and facility-specific policies.
- Active participation in ongoing training and competency assessments is required.
- Candidates with religious or medical exemptions for vaccines or mask fit testing must complete specific attestation forms and obtain approval.
