Description
Registered Nurse - Adult Care
Location: New York, NY
Employment Type: Community Care - Nursing
Contract Length: 13 weeks
Shift: Home care position, providing care in patients' homes
Hours: Approximately 7 hours per day, 37.5 hours per week
Schedule: Every other weekend required
Patient Load: 4-8 patients per day
Job Responsibilities
- Assess, plan, implement, and evaluate patient care plans in consultation with healthcare professionals
- Provide direct, individualized bedside nursing care to pre- and post-surgery patients
- Prepare, administer, and document prescribed medications
- Change dressings, insert catheters, and start IVs
- Adjust specialized medical equipment and interpret measurements
- Monitor patient vitals and report adverse medication reactions promptly
- Collaborate with healthcare team on discharge planning
- Educate patients and families on surgical procedures and post-surgery care
- Maintain and update patient files, medical charts, and health records
- Adhere to best nursing practices, standards, and protocols
Required Qualifications
Experience
- Minimum 1 year of Home Health experience within the last year
- Experience in Acute Care, Post Acute, and Home Care settings
- Med Surg experience
- Telemetry monitoring experience
Education
- Associate of Science in Nursing or Associate Degree in Nursing from an accredited institution
Certifications & Licenses
- Valid New York State RN License
- American Heart Association Basic Life Support (BLS) certification
Preferred Qualifications
- Experience with EPIC Electronic Medical Records (EMR) system
Additional Information
- This is a true home care nursing position requiring experience in providing care in patients' homes
- Transportation support (e.g., Lyft rides) available within the borough
