Description
Case Manager (RN Case Manager)
Location: Cambridge, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: 8:00 AM – 4:30 PM
Job Summary
- Coordinate and support healthcare services within the facility and referrals outside the facility.
- Provide outreach and enrollment services to meet program eligibility requirements.
- Develop and coordinate community service plans involving patients, families, and service providers.
- Conduct client assessments and modify patient treatment plans based on patient responses and conditions.
- Prepare patients and families for discharge.
- Communicate with third-party payers to obtain necessary authorizations for service reimbursement.
- Collaborate with the medical team on challenging cases.
- Maintain accurate and detailed reports and records.
- Educate and guide patients and families to define care goals.
Required Qualifications
- Valid RN license in Massachusetts.
- Minimum 2 years of case management experience, including experience within the last year.
- BLS certification issued by the American Heart Association or Red Cross.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Preferred Qualifications
- Hepatitis B vaccination or proof of immunity (strongly preferred).
- Tdap vaccination within the last 10 years (preferred).
Notes
- No religious or medical exemptions allowed for N95 fit testing.
- Fit testing cannot be performed onsite; must be completed prior to start.
- If positive TB test, documentation of a negative chest x-ray is required.
- Name change documentation required if credentials do not match current legal name.
This position requires strong coordination skills, attention to detail, and the ability to work collaboratively with patients, families, and healthcare teams to ensure comprehensive case management and discharge planning.
