Description
Registered Nurse - Home Health (Travel/Contract)
Location: Burlington, North Carolina
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Hours per Week: 40
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Patient visits include start of care, wound care, follow-up visits, and other home health nursing tasks.
- Average patient load: 5-8 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day (30 points per week expected).
- Visits occur both in patients' homes and care facilities.
- Must be willing to travel up to 75 miles from the home clinic; typical travel is about 30-40 miles per day.
- Reliable transportation, current driver’s license, and agency-required liability insurance are mandatory.
- Charting and documentation are done using HomeCare HomeBase (HCHB) and Oasis EMR systems. Proficiency with these systems within the last 6 months is required; retraining may be required if proficiency is outdated.
- Dress code: Scrubs.
Required Qualifications
- Valid Registered Nurse (RN) license in North Carolina.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum of 1 year of recent Home Health experience within the last year, including experience working with adult patients.
- Hospice experience.
- Current BLS certification issued by the American Heart Association or American Red Cross.
- Experience with HomeCare HomeBase (HCHB) documentation system.
Additional Details
- Weekend and on-call requirements vary by location and should be discussed during the interview.
- No guaranteed hours; if patient census is low, floating to a nearby clinic may be arranged to pick up hours.
- Local candidates are accepted under the same terms as travelers.
- Contract may lead to potential hire at assignment end.
- Territory coverage includes Guilford County, Rockingham County, and Alamance County in North Carolina.
Compliance and Documentation Requirements
- Must complete all required background checks including but not limited to:
- Standard 7-year background check with SS trace, county, nationwide, sex offender, and state-specific searches.
- EPLS/GSA/SAM and HHS/OIG checks.
- Fingerprinting as required by state (NC SBI fingerprinting mandatory, cannot start with results pending).
- Health requirements include:
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
- Drug screening (10-panel).
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination.
- Must review and sign all required compliance documents prior to start, including but not limited to:
- Confidentiality and privacy policies.
- Code of conduct and compliance training.
- Home Health Core Policy and scenarios.
- HIPAA and privacy policies.
- COVID-19 policy and attestation.
References and Work History
- Two professional references required.
- Verification of at least two most recent employers or last 7 years of work history.
This position is ideal for experienced RNs with recent home health or hospice experience who are comfortable with travel and working independently in a home care setting.
