Job Opportunity: Case Manager (Registered Nurse)
Location: Oakland, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
Seeking an experienced Registered Nurse Case Manager with 5+ years of inpatient acute case management experience. The role involves care coordination and care transitions planning throughout the acute care patient experience, working collaboratively with physicians, utilization managers, social workers, bedside nurses, and other interdisciplinary team members to ensure timely and appropriate patient care transitions and resource utilization.
Work Schedule
- Shift: Day shift (5 days a week, 8 hours per day)
- Hours: 8:00 AM – 4:30 PM
- Every other weekend required
- 5/8s schedule
Key Responsibilities
- Conduct initial and ongoing patient assessments, including medical, psychosocial, and financial information gathering.
- Develop and lead comprehensive, cost-effective care coordination plans.
- Oversee Case Management Assistants in determining post-acute care preferences.
- Perform utilization management by reviewing medical records to ensure appropriate level of care (LOC) and expedite transitions.
- Collaborate with physicians and interdisciplinary teams to prevent unnecessary admissions/readmissions.
- Monitor length of stay and identify outliers requiring additional focus.
- Ensure compliance with Medicare and regulatory requirements related to care transitions.
- Formulate and communicate transition plans involving patients, families, payers, and community resources.
- Identify barriers to care progression and work with the team to resolve them promptly.
- Address complex clinical and social situations efficiently, including high utilizers in the Emergency Department.
- Maintain thorough documentation in electronic health records (EHR).
- Participate actively in department operations and continuous improvement initiatives.
- Foster positive, professional relationships with healthcare teams and community agencies.
Required Qualifications
- Valid Registered Nurse (RN) license in California
- Minimum 5 years of Care Manager experience, including acute care case management in an inpatient setting
- Care Manager experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Background and Compliance Requirements
- 7-year background check including SSN trace, county searches, sex offender search, education verification, and FACIS Level 3 screening
- Drug screening: Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids per California law)
- Physical exam within 1 year of start, including clearance for work
- Immunizations and health screenings:
- Adult TDAP within 10 years or lifetime TDAP with booster
- Negative TB test or chest X-ray within 90 days, renewed annually
- Measles, Mumps, Rubella, Varicella, Hepatitis B immunity or vaccination series (with exemptions considered)
- Seasonal flu vaccination during flu season (Nov 1 – Apr 30) or signed declination
- Fit mask test within 1 year, renewed annually (with accommodations for religious exemptions or failed tests)
- Respiratory safety clearance prior to start, renewed annually
- Completion of facility-specific documents including confidentiality agreements, code of conduct, and regulatory orientation prior to start
Additional Requirements
- Full 7 years of verifiable work history with no gaps over 30 days
- Professional references as required by agency (not required by client)
- Competency exams as applicable (medication/pharmacology, dysrhythmia/EKG if applicable)
- Completion of vendor training and other mandatory facility trainings prior to start
