Case Manager (Registered Nurse) - Travel/Contract
Location: Williamsburg, Virginia
Contract Length: 13 weeks
Job Description
- Home hospice case manager role
- Must have experience specifically with home hospice case management (no exceptions)
- Patient census approximately 35-45 patients
- Work shift: Day shift, 5 days a week, 8 hours per day (10:00 AM to 6:00 PM)
Required Qualifications
- Valid Registered Nurse (RN) license in Virginia
- Minimum of 1 year of case management experience, including experience within the last year
- Experience specifically in home hospice case management
- Experience in a Home Health setting
- Hospice experience
- Current Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross (online CPR not accepted)
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Experience with Epic electronic health record system
- Government-issued photo ID and valid driver’s license required (minimum 3 years driving experience, minimum age 21; 25 if transporting patients)
- Must pass a 7-year criminal background check, including motor vehicle/driving record review
- Must complete facility-required competency exams and pass/fail testing as applicable
- Must complete required facility-specific acknowledgements and policies prior to start (e.g., confidentiality, dress code, safety, drug-free policy)
- Must provide 2 professional references
- Application/resume required
Health and Safety Requirements
- Physical exam within the last 12 months
- Negative TB test or chest X-ray per facility protocol; if positive TB, clearance letter signed by a physician required
- Required vaccinations or proof of immunity (or approved exemptions) for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B (series, titer, or declination)
- Seasonal flu vaccination required during flu season (Oct 1 - Mar 31) with strict deadlines; medical or religious exemptions accepted with approval
- COVID vaccine series preferred; declination form accepted if no vaccine records available
- Drug screen within the last 12 months covering a comprehensive panel of substances
Documentation and Compliance
- Photo for facility badge (recent, professional, per detailed photo requirements)
- Virginia State Police Check fingerprint results required prior to start
- E-Verify or attestation required
- Facility-specific forms and policies must be reviewed and signed before starting
- Annual updates required for some documents and competencies
