Case Manager (Registered Nurse) - Travel/Contract
Location: Oakland, California
Contract Length: 13 weeks
Job Details
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
- Schedule: Every other weekend off
- Workweek: 40 hours
Job Summary
Responsible for care coordination and care transitions planning throughout the acute care patient experience. Collaborates with physicians, utilization managers, social workers, bedside nurses, and interdisciplinary teams to ensure timely patient progression and appropriate level of care. Focuses on preventing unnecessary admissions or readmissions by facilitating communication and resource coordination across the care continuum.
Key Responsibilities
- Conduct initial and ongoing patient assessments including medical, psychosocial, and financial information.
- Lead healthcare team in developing cost-effective care coordination plans.
- Oversee case management assistants for post-acute care service preferences.
- Review medical records to ensure patients meet level of care (LOC) requirements and support documentation.
- Expedite transition planning for patients no longer requiring acute care.
- Monitor length of stay and identify outliers needing additional resources.
- Collaborate with financial counselors regarding inpatient stay denials and uninsured/underinsured patients.
- Ensure compliance with Medicare Important Message delivery timelines.
- Participate in patient rounds and interdisciplinary team meetings to assure timely transitions.
- Maintain documentation in electronic health records (EHR) and other software.
- Stay current with CMS, Joint Commission, and regulatory requirements related to transitions of care.
- Formulate and communicate transition plans involving patients, families, physicians, payers, and community resources.
- Screen for 30-day readmissions and identify barriers to care progression.
- Address complex clinical and social situations to avoid unnecessary admissions and delays.
- Identify high utilizers in the Emergency Department and make appropriate referrals.
- Participate actively in department operations and continuous improvement initiatives.
- Maintain professional relationships with healthcare team and community agencies.
Required Qualifications
- Valid Registered Nurse (RN) license in California
- Minimum 5 years of case management experience, including acute hospital case management
- Case management experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Experience in an acute care setting
Background and Compliance Requirements
- 7-year background check including SSN trace, county searches, sex offender, education verification, and FACIS Level 3 screening.
- Annual updates required for EPLS/GSA/SAM, HHS/OIG, and FACIS screenings.
- Education verification required; Bachelor of Science in Nursing preferred.
- Drug screening: Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids per California law).
- Physical exam within 1 year of start.
- TB screening: Negative test or chest x-ray within 90 days, renewed annually.
- Immunizations and titers required:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Measles (Rubeola), Mumps, Rubella (MMR)
- Hepatitis B series or declination
- Varicella
- Seasonal flu vaccination during flu season (Nov 1 - Apr 30) or signed declination
- Fit mask test within 1 year, renewed annually (with accommodations for religious exemptions or failed tests).
- Respiratory safety clearance required annually.
- Completion of facility-specific documents including code of conduct, confidentiality agreements, HIPAA, and orientation materials prior to start.
- Employment application must include full 7 years of work history with no gaps over 30 days.
- Professional references required per agency policy (not required by client).
