Case Manager

Overview

Travel/contract role in San Francisco, California
Primarily virtual, supporting complex cases across the system and patients in the community
May float to offices in San Francisco and Emeryville as needed
Day shift: 5x8s, 8:00 AM to 4:30 PM
40 hours per week
Contract length: 13 weeks

Required Qualifications

Valid RN license in California
Case Management (1 year of experience)
Case Management experience within the last year
Associate of Science in Nursing / Associate Degree in Nursing
Experience in an Inpatient setting

Schedule / Work Setting

Primarily virtual
May need to float between office locations in San Francisco and Emeryville
