Description
RN Case Management / Discharge Planning
Location
- Jackson, Mississippi
Schedule
- Days, Monday through Friday
- 8-hour shifts
- No weekend requirement
- Holidays rotated as needed
Assignment Length
- 13 weeks
Required Qualifications
- Valid RN license in Mississippi
- Case Management (2 years of experience)
- Case Management experience within the last year
- One completed travel assignment
- BLS issued by American Heart Association
- Associate of Science in Nursing / Associate Degree in Nursing
- Experience in a(n) Inpatient setting
- Acute Hospital Case Management Experience
- Discharge Planning
- Experience with Epic
Preferred Qualifications
- CCM certification
- BSN preferred
- ADN may be considered
Work Setting / Scope
- Covers all inpatient units throughout the hospital
- Typical caseload: 15–20 patients per day
- Works with the social work team
- May consult with respiratory therapy and pharmacy as needed
Additional Requirements
- Flu vaccine required; only medical exemption with documented evidence of a past severe reaction is allowed
- Fingerprint clearance required before start
- Submission must include trauma level, bed size, and specific duties for each job in the past 3 years
- Previous travel experience in a Level 1 facility within the last 3 years is required
Orientation
- 2 weeks of department-specific orientation, including general facility orientation
- Minimal unit training; candidate should be able to learn quickly and work efficiently
Dress Code
- Any color scrubs or business professional attire
