Overview

Required Qualifications

• Valid RN license in Massachusetts
• Associate of Science in Nursing / Associate Degree in Nursing
• Minimum 2 years of recent (within 12 months) clinical experience providing in-person triage and direct patient care for acute and chronic conditions in an ambulatory outpatient setting
• Clinic experience within the last year
• Experience in an Ambulatory Care setting
• Experience with telephone triage and telephone-based clinical assessment techniques
• Gastroenterology experience
• Endocrinology experience
• BLS issued by American Heart Association or American Red Cross

• MMR documentation or proof of immunity
• Influenza vaccine

• Health statement signed by an MD, DO, NP, or PA
• Criminal background check
• Massachusetts CORI

Notes

• MA Conditional Licenses are not accepted
• All work experience and education must include city and state
• Positive TB test or titer requires a chest x-ray showing no active tuberculosis
• Name change documentation is required if credential documents do not match the candidate’s current legal name or profile name
• The flu mist is accepted only if administered directly by a pharmacy or provider; self-administered flu mist is not accepted
