RN Case Mgmt
Overview
- Travel RN case management role in Paramount, California.
- Schedule: Monday–Friday, 8:00 AM–4:30 PM.
- Contract length: 13 weeks.
Required Qualifications
- Valid California RN license.
- Associate of Science in Nursing / Associate Degree in Nursing.
- BLS issued by the American Heart Association.
- 2 years of Case Management experience.
- Case Management experience within the last year.
- Experience in an Acute Care or Long Term Acute Care setting.
- Experience with utilization review (InterQual).
- Experience with discharge planning.
Role Summary
- Coordinates care management, utilization review, discharge planning, and resource utilization.
- Supports comprehensive assessment, care planning, implementation, and evaluation of patient needs.
- Serves as a patient advocate and supports continuity of care and cost effectiveness.
