Required Qualifications
- Valid RN license in Massachusetts
- Home Health experience within the last year
- Associate of Science in Nursing / Associate Degree in Nursing
- Driver's License
- Case Management experience
Responsibilities
- Provide clinical services to patients from birth to over 100 years old in their homes
- Assess patient conditions, implement care plans, and provide education and support to patients and families
- Perform admissions and case management in accordance with policies and standards of practice
- Use timely electronic documentation and interdisciplinary communication

- Weekend rotations occur once every 6 weeks
