Case Manager RN

Overview

Travel/Contract role in Oakland, California
Day shift: 5x8s, 8:00–16:30
Every other weekend
13-week contract

Required Qualifications

Valid RN license in California
Associate of Science in Nursing / Associate Degree in Nursing
5 years of Case Management experience
Case Management experience within the last year
Experience in an Acute Care setting
Experience in Leadership roles
Experience with discharge planning
Experience with acute hospital case management

Role Summary

Responsible for care coordination and care transitions planning throughout the acute care patient experience
Collaborates with physicians, utilization management, social work, bedside RNs, and other members of the interdisciplinary team to support timely transitions of care and appropriate level-of-care placement
If assigned to the Emergency Department, addresses complex clinical and social situations to help avoid unnecessary admissions
