Overview

• Travel/Contract role in Marietta, Ohio
• Home Health assignment
• Contract length: 13 weeks
• Shift: Day, 5x8 hours, 8:00 AM–5:00 PM
• Schedule: Monday–Friday
• No weekend rotation
• No on-call

Setting and territory

• Home health
• Territory: Washington, Athens, and Meigs counties
• Report location: Marietta, Ohio
• Average weekly travel: 250–300 miles
• Must be willing to travel up to 75 miles from the home clinic
• Locals are accepted at the same rate

Patient care and workload

• Provide home visits and attend periodic office meetings
• Average daily caseload: 5 to 8 patients
• Weekly productivity expectation: 30 points
• Case types may include:

  • Start of Care
  • Wound Care
  • Follow-up visits
  • Other adult home health or hospice services
• No guaranteed hours
• If patient census is low, may float to a nearby clinic for additional hours

Required qualifications

• Valid RN license in Ohio
• Associate of Science in Nursing / Associate Degree in Nursing
• One year of Home Health experience
• Home Health experience within the last year
• Hospice experience
• One completed travel assignment
• BLS
• Driver's License
• Experience with Homecare Homebase
• Experience with OASIS
• Reliable transportation
• Active license in hand; pending licenses are not accepted
• Agency-required liability insurance

Charting and documentation

• Homecare Homebase (HCHB) proficiency required
• Must have used HCHB within the past six months
• OASIS proficiency required for RNs

Preferred / additional notes

• Prior HCHB experience outside the six-month window may be considered, but retraining is required during compliance
• Dress code: Scrubs
