RN Care Coordinator (Case Management)
Overview
- RN Care Coordinator (Case Manager) in Boston, Massachusetts
- Contract length: 13 weeks
- Shift: 07:00 - 15:30
Role Summary
- Manages a caseload of patients and coordinates care to support clinical and financial outcomes.
- Uses patient assessment, care guidelines, protocols, payer regulations, and response to therapies to assess the episode of illness from pre-admission to post-discharge.
- Participates in evaluating practice patterns and systems and supports quality, cost, and satisfaction improvement efforts.
- Mobilizes resources to maximize efficiency of care delivery.
Required Qualifications
- Valid RN license in Massachusetts
- Case Management experience within the last year
- 1 year of Case Management experience
- Associate of Science in Nursing / Associate Degree in Nursing
