Position Details

• Location: Valley City, North Dakota
• Contract Length: 13 weeks

Requirements

• Certifications Required:

  • Basic Life Support (BLS)
  • North Dakota Physical Therapy License
• Experience:

  • Minimum of 1 year of PT-related skills and previous home health experience
• Driver's License & Auto Insurance: Must have a current driver's license and auto insurance

Patient Care

• Number of Patients Treated per Day: 5-7
• Common Diagnoses/Types of Patients: Home health clients including those recovering from surgeries, falls, etc.
• Patient Coverage Area: 60-mile radius around Valley City, ND
• EMR System: HomeCare/HomeBase

Work Schedule

• Shift Details: Monday to Friday, 8 AM - 5 PM
• Weekend Requirements: None
• Holiday Requirements: None
• Dress Code: CSH color scrubs or business casual
• Orientation: 3-5 shifts of unit/department-specific orientation

Additional Information

• No Clinical Involvement/Manager Interview: Candidates will not have clinical involvement or a manager interview.
• Radius Rule: Candidates may not have been employed full-time, part-time, or PRN with any facility associated with CommonSpirit (including CHI and/or Dignity) within the past 12 months.
• Rehabilitation Staff: Possibility of staffing PTAs (Physical Therapist Assistants).
• RTO (Request Time Off): Must be provided upfront; additional RTO will not be approved after submission.
