Job Description

• Provide care to adult medical/surgical inpatients, including orthopaedic, neuro-surgery, and level 1 trauma patients.
• Focus on zero-harm, patient safety, and quality care.
• Enhance patient experience through pain management, proactive rounding, and bedside handoff.

Required Credentials & Certifications

• BLS (Basic Life Support) – American Heart Association only, required for clinical positions.
• License/Certificate Verification – Valid permanent RN license required; temporary licenses not accepted. Must be verified within 30 days of start and updated every 2 years with license renewal.
• MMR (Measles, Mumps, Rubella) – Positive titers within last 15 years required; booster protocol applies if low titer.
• Fit Test – Completed within 1 year of start, updated annually.
• Hepatitis B – Proof of full series, partial series with agency form, or positive titers within 15 years.
• Varicella – Positive titers within last 15 years required; booster protocol applies if low titer.
• Tdap – Proof of vaccination within past 10 years, no declinations accepted.
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – Negative test within last 6 months required; annual requirement. If positive, clear chest x-ray and current TB questionnaire required.
• Physical/Health Statement – Pre-employment physical exam within 1 year stating ability to work without restrictions.
• UHS Competency Exam – Must score at least 80%, completed within 1 year prior to start.
• UHS Core Competencies – Includes HIPAA, Joint Commission-NPSG, OSHA topics; 80% passing score required, completed within 1 year prior to start.
• UHS Skills Checklist – Clinical skill-specific checklist completed within 1 year prior to start (initial hire only).
• UHS I-9 Attestation – Proof of eligibility to work in the U.S.
• UHS Ohio Fingerprints – Ohio BCI&I check and FCRA-compliant national criminal background check required. Attestation allowed if fingerprints pending.
• UHS Criminal Background Check Results – Completed within 6 months of start, including SSN trace and national criminal sweep.
• UHS OIG Search – Completed within 6 months prior to start (initial hire only).
• UHS SAM/EPLS/GSA/OFAC – Screening completed within 6 months prior to start (initial hire only).
• UHS Drug Screen – 10-panel drug screen completed within 60 days of start; instant drug screens not accepted.
• UHS Education Verification – Highest level of education verified; international education requires equivalency documentation.
• UHS Employment Verification – Verification of employment within last 5 years through third-party verification.
• UHS Submission Packet – Resume including education and work history, 2 professional references within last 2 years, and completed job application.
• UHS Badge Photo – Professional headshot required.
• Driver's License/State ID – Non-expired photo ID required.

Preferred / Optional Credentials

• ACLS (Advanced Cardiovascular Life Support) – American Heart Association only, optional.
• COVID-19 Vaccine – Proof or approved exemption preferred but not required.
• Influenza Vaccine – Required during flu season (Oct-Mar) or approved medical/religious exemption.
• Annual Negative PPD – If 2-step PPD not done within past year.
• Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire – If applicable.
• UHS Social Security Trace – If not included in background check, to be completed within 1 year of start.
• UHS NSO (National Service Office) – If not included in background check, to be completed within 30 days of start.
• UHS National Database Criminal Background Check – For Ohio residents to meet fingerprint requirements.

Additional Notes

• Attestations are generally not accepted except for fingerprinting pending status.
• All required documentation must be uploaded and verified prior to onboarding.
• Compliance with all credentialing and background checks is mandatory for assignment start.
