Required Credentials & Certifications

  • Government Issued Photo ID: Driver's License or State ID
  • ACLS (Advanced Cardiac Life Support): AHA Provider/Instructor/RQI acceptable, as required by unit
  • BLS (Basic Life Support): AHA Provider/Instructor/RQI acceptable, as required by unit
  • PALS (Pediatric Advanced Life Support): AHA Provider/Instructor/RQI acceptable, as required by unit
  • NRP (Neonatal Resuscitation Procedure): As required by unit
  • TNCC (Trauma Nursing Core Course): As required by unit
  • EFM (Electronic Fetal Monitoring): As required by unit
  • Tech Cert Verification - National: As required within 30 days of start
  • HIPAA Safety Manual Acknowledgement: Required
  • State Nursing License Verification: Primary source verification within 30 days of start
  • NURSYS Verification (RN, LPN/LVN, AP): Required within 30 days of start
  • NIH Stroke Certification: As required by unit, updated annually

Competencies

  • Medication Test/Pharmacology Competency Exam (as required)
  • Color Blind Testing (one-time requirement)
  • Competency Exam (as required by specialty)

Facility Specific Requirements

  • Passport-style photo for badge
  • Completion of Rochester Regional Orientation Modules (4-6 hours) prior to start
  • Pre-Employment Compliance Attestations and New Hire Forms

Health & Immunization Requirements

  • Physical Exam: Within 1 year of start, updated annually
  • Fit Mask Test: Within 1 year of start, updated annually (specific mask types accepted)
  • TB Testing: Negative Qgold, Tspot, or 2-Step PPD within 1 year; if positive, negative chest X-ray within 10 years plus annual TB questionnaire
  • TB Questionnaire: Required in lieu of annual TB test renewal
  • Rubeola (Measles): Positive titer or 2 vaccines; boosters required for low/equivocal titers; age exemption if born before 1957
  • Mumps: Positive titer or 2 vaccines; boosters required for low/equivocal titers; age exemption if born before 1957
  • Rubella: Positive titer or 1 MMR/Rubella vaccine; booster required for low/equivocal titers; age exemption if born before 1957
  • Hepatitis B: Positive surface antibody titer, completed series, or declination; dialysis candidates require additional titers
  • Varicella: Positive titer, 2 vaccines, or documented history; boosters or declination accepted for low/equivocal titers
  • Drug Testing: Required within 30 days of start; includes amphetamines, cocaine, ecstasy, fentanyl, heroin, opiates
  • Flu Vaccination/Documentation: Required during flu season or signed declination
  • Eye Exam: Snellen exam within 1 year of start (not required annually)
  • COVID Vaccination/Documentation: Copy preferred but not required

Background & Employment History

  • 7-Year Criminal Background Check: Within 3 months of start, including SSN trace, national criminal, sex offender, education verification, eligibility to work, and international search if applicable
  • EPLS/GSA/SAM Checks: Required within 3 months of start and monthly thereafter
  • HHS/OIG Checks: Required within 3 months of start and monthly thereafter
  • References: Minimum of 2 employment references required
  • Employment Application: Required
  • Skills Checklist: Required annually
  • Foreign Education Evaluation: Required if education was obtained internationally and no verification on file
  • Transcript/Diploma: Required if no education verification on file
  • Work History Verification: Must verify at least 2 past employments, including dates, location, role, and employment type
  • Education Verification: Academic verification from a third party required

State-Specific Requirements (New York)

  • NYS Nurse Aide Registry: Required within 3 months of start for Long Term Care, ElderOne, Home Care, and Hospice Care
  • NYS Criminal Background Check Fingerprint Results: Required for Mental Health, Behavioral Health, Home Health Aide, and LTC units (facility to confirm)
  • Disciplinary Action Search Results: Required for Mental Health, Behavioral Health, Home Health Aide, and LTC units (facility to confirm)
  • Sex Offender Registry Search: Required within 30 days of start
  • NYS Medicaid Exclusion List Check: Required within 3 months of start and monthly thereafter
