Job Description

• Position involves moving to the correct facility from MSP flip.

Required Credentials & Certifications

• Government Issued Photo ID: Driver's License or State ID required.
• Certifications (AHA ONLY, Provider/Instructor/RQI acceptable as per unit):

  • ACLS (Advanced Cardiac Life Support)
  • BLS (Basic Life Support)
  • PALS (Pediatric Advanced Life Support)
• Other Certifications (as required by unit):

  • NRP (Neonatal Resuscitation Procedure)
  • TNCC (Trauma Nursing Core Course)
  • EFM (Electronic Fetal Monitoring)
  • NIH Stroke Certification (annual update)
• Verification Requirements:

  • State License Primary Source Verification within 30 days of start
  • NURSYS Verification for RN, LPN/LVN, AP within 30 days of start
  • Tech Cert Verification (national) within 30 days of start if applicable
• Compliance:

  • HIPAA Safety Manual Acknowledgement required

Competencies

• Medication Test/Pharmacology Competency Exam (as required by profession)
• Color Blind Testing (required once)
• Competency Exam (as required by profession/specialty)

Facility Specific Requirements

• Passport-style photo for badge
• Completion of Rochester Regional Orientation Modules (4-6 hours, prior to start)
• Completion of Pre-Employment Compliance and Health Records Attestations
• New Hire Form

Health & Immunization Requirements

• Physical Exam within 1 year, updated annually
• Fit Mask (specific models accepted), updated annually
• Negative TB test or Chest X-Ray within 1 year; TB Questionnaire required annually or in lieu of test
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (Surface AB positive titer, completed series or declination)
  • Varicella
• Drug Testing within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
• Seasonal Flu Vaccination or signed declination during flu season
• Snellen Eye Exam within 1 year (not required annually)
• COVID-19 Vaccination documentation preferred but not required

Background & Employment History

• 7-year Criminal Background Check within 3 months of start (includes SSN trace, national criminal, sex offender, education verification)
• EPLS/GSA/SAM checks within 3 months of start, run monthly
• Minimum of 2 employment references required
• Employment Application required
• Annual Skills Checklist
• Foreign Equivalency Education Evaluation if education obtained internationally
• Transcript/Diploma verification if no education verification on file
• Work History Verification for at least 2 past employments
• Academic Education Verification from 3rd party required

State-Specific Requirements (New York)

• NYS Nurse Aide Registry (within 3 months of start for LTC, ElderOne, Home Care, Hospice)
• NYS Criminal Background Check Fingerprint Results (for Mental Health, Behavioral Health, Home Health Aide, LTC units as requested)
• Disciplinary Action Search Results (for Mental Health, Behavioral Health, Home Health Aide, LTC units as requested)
• NY Sex Offender Registry Search Results within 30 days of start
• NY Medicaid Exclusion List check within 3 months of start, run monthly
