Job Opportunity: Registered Nurse (Travel/Contract)

Location: Rochester, New York
Facility: Rochester Culver Road, The NeuroMedical Center


Contract Details

Required Credentials & Certifications

  • Government Issued Photo ID: Driver’s License or State ID
  • Licenses:
    • State License (Primary Source Verification within 30 days of start)
    • NURSYS Verification for RN, LPN/LVN, AP (within 30 days of start)
  • Certifications (AHA ONLY, Provider/Instructor/RQI acceptable):
    • ACLS (Advanced Cardiac Life Support)
    • BLS (Basic Life Support)
    • PALS (Pediatric Advanced Life Support)
  • Additional Certifications (as required by unit):
    • NRP (Neonatal Resuscitation Procedure)
    • TNCC (Trauma Nursing Core Course)
    • EFM (Electronic Fetal Monitoring)
    • NIH Stroke Certification (annual update)
  • Competencies:
    • Medication Test/Pharmacology Competency Exam (as required)
    • Color Blind Testing (one-time)
    • Competency Exam (as required by specialty)
  • HIPAA Safety Manual Acknowledgement (required)
  • Tech Cert Verification - National (if applicable, within 30 days of start)

Health & Immunization Requirements

  • Physical Exam: Within 1 year of start, updated annually
  • Fit Mask: Required annually (specific acceptable masks listed)
  • TB Testing: Negative Qgold, Tspot, or 2-Step PPD within 1 year; if positive, additional documentation required
  • TB Questionnaire: Required in lieu of annual TB test renewal
  • Immunizations:
    • Rubeola (Measles): Positive titer or 2 vaccines (age exemption if born before 1957)
    • Mumps: Positive titer or 2 vaccines (age exemption if born before 1957)
    • Rubella: Positive titer or 1 MMR/Rubella vaccine (age exemption if born before 1957)
    • Hepatitis B: Positive titer, completed series, or declination (additional for Dialysis)
    • Varicella: Positive titer, 2 vaccines, or documented history
  • Drug Testing: Required within 30 days of start; includes multiple substances
  • Seasonal Vaccinations:
    • Flu vaccination or signed declination (deadline 12/3)
    • COVID vaccination documentation preferred but not required
  • Eye Exam: Snellen exam within 1 year of start (not annual)

Background & Compliance Checks

  • Background Check: 7-year criminal background check within 3 months of start
  • EPLS/GSA/SAM Checks: Required within 3 months of start and monthly thereafter
  • HHS/OIG Check: Required within 3 months of start and monthly thereafter
  • References: Minimum of 2 employment references required
  • Employment Application: Required
  • Skills Checklist: Required annually
  • Education Verification: Academic verification from a 3rd party required
  • Work History Verification: At least 2 past employments verified

Facility-Specific Requirements

  • Photo for Badge: Clear, passport-style photo with solid background
  • Orientation Modules: Must be completed prior to start (approx. 4-6 hours)
  • Pre-Employment Compliance Attestations: Required
  • New Hire Form: Required

State-Specific Requirements (New York)

  • NYS Nurse Aide Registry: Required within 3 months for certain care areas
  • Criminal Background Check Fingerprint Results: Required for Mental Health, Behavioral Health, Home Health Aide, and LTC units (if applicable)
  • Disciplinary Action Search Results: Required for certain units (Mental Health, Behavioral Health, Home Health Aide, LTC)
  • Sex Offender Registry Search: Required within 30 days of start
  • NYS Medicaid Exclusion List Check: Required within 3 months of start and monthly thereafter

This position requires thorough credentialing, health documentation, and compliance with both facility and state-specific regulations. Candidates should be prepared to complete all required documentation and certifications prior to starting the assignment.
