Travel RN Medical/Surgical Position
Location: Milwaukee, Wisconsin

Job Description
- Every Other Week (EOW) and Every Other Holiday (EOH) schedule
- Requires 2 or more years of medical/surgical experience
- Telemetry experience required
- Experience with EPIC electronic health record system
- May need to float onsite within skill set or to other sites within 40 miles if needed
Required Certifications & Licenses
- BLS (American Heart Association)
- Current nursing license with Nursys verification within 30 days prior to start
- License/Certificate verification within 30 days prior to start
- Driver’s License/State ID (must be current and valid)
Required Training & Exams
- Ascension Color Vision test (completed prior to start, updated every 3 years)
- Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start, updated yearly, minimum 80% score)
- Ascension EKG Exam (passing score of at least 80%, two attempts allowed)
- Ascension Epic Test-Out (Epic test results and training checklist with signed attestation)
- Ascension Specialty Exam (completed prior to first assignment, updated every 3 years, minimum 80% or national benchmark passing score)
- Ascension Skills Checklist (clinical skills checklist, completed within one year of start, updated annually)
- Ascension 12 Panel Drug Screen (completed within 30 days prior to start, updated annually)
Onboarding & Professional Requirements
- Ascension Onboarding Requirements Education Attestation
- Ascension Nursing Contract Worker Onboarding (done onsite)
- Ascension Nova StatStrip (pre-hire modules for non-allied positions)
- Ascension Badge Photo (forward-facing headshot with white or blue background, no alterations)
- Ascension Profile (resume or professional profile)
- Ascension 2 Professional References (at least two professional references)
- Ascension Criminal Background Check (including Social Security Trace, Employment Verification, OIG Search, National Sex Offender; completed within 30 days of first assignment, updated annually)
- Ascension EPLS (Exclusionary Party List System check, run within 90 days of start, rerun if break in service > 90 days)
- Ascension Education Verification (verification of education relevant to clinical work, completed within 90 days of start)
- Ascension Confirmation of Assignment (documented start and end dates on agency letterhead)
- Ascension Employment Verification (third-party employment verification for past 7 years, updated annually)
Health & Immunization Requirements
- MMR: Documented proof of two MMR vaccines or positive IGG titer; no declinations accepted
- Fit Test: Required for all travelers, preferred prior to start, updated annually
- Hepatitis B: Documentation of vaccination series completion or signed declination
- Influenza Vaccine: Required for flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
- Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire (preferred, optional)
- Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within 30 days prior to start, updated annually
- Varicella: Documented proof of two vaccines, serological evidence, or history verified by healthcare provider; no declinations accepted
- Tdap: Valid within 10 years, no declinations accepted
Additional Notes
- Nursys verification required within 30 days prior to start; disciplinary actions require FACIS Questionnaire
- Some documents and exams require uploading prior to or at onboarding
- Floating may be required within skill set or to nearby sites (within 40 miles)
- Must meet all Ascension-specific onboarding and competency requirements
This position is ideal for experienced Med/Surg RNs with telemetry and EPIC experience who are comfortable with travel assignments and meeting comprehensive credentialing and onboarding requirements.
