Job Description

• Role involves intraprocedural circulating and documentation using EPIC Cupid system.
• Responsibilities include patient preparation and recovery.
• Schedule: Monday through Friday with on-call duties as needed for STEMI and some interventional radiology (IR) cases.

Required Certifications & Licenses

• BLS (American Heart Association only)
• ACLS (American Heart Association only)
• Valid permanent RN license (no temporary licenses accepted) with primary source verification within 30 days of start; must be updated every 2 years with license renewal.

Required Health & Safety Credentials

• COVID-19 Vaccine (preferred, not required)
• Positive MMR titers within last 15 years or appropriate booster protocol
• Fit Test completed within 1 year of start
• Hepatitis B vaccination series or signed declination/positive titers within 15 years
• Positive PPD: Negative chest X-ray within 5 years and current TB questionnaire
• Positive Varicella titers within last 15 years or appropriate booster protocol
• Tdap vaccination within past 10 years (no declinations accepted)
• 2-Step PPD or IGRA (Quantiferon/Tspot) within last 6 months (annual requirement)
• Influenza vaccine (Oct-March) or approved medical/religious exemption (no declinations)
• Physical/Health Statement completed within 1 year prior to start confirming ability to work without restrictions
• Drug Screen within 60 days prior to start (10-panel, no instant screens)

Professional & Compliance Requirements

• Completed I-9 attestation confirming eligibility to work
• Ohio fingerprinting and criminal background checks compliant with FCRA standards (requirements differ based on length of Ohio residency)
• Education verification (highest level, no attestations; equivalency required for international education)
• Employment verification for last 5 years through third-party verification (no attestations)
• Criminal background check including SSN trace, county searches, national criminal sweep, and NSO within 6 months prior to start
• OIG exclusion search within 6 months prior to start
• SAM/EPLS/GSA/OFAC clearance within 6 months prior to start
• Competency exam for relevant department with minimum 80% score within 1 year prior to start
• Core competencies (HIPAA, Joint Commission, OSHA topics) completed within 1 year prior to start with minimum 80% score
• Skills checklist specific to clinical area completed within 1 year prior to start (initial hire only)
• Module completion certificate for safety and compliance training prior to clearance
• Submission packet including resume, education, work history, and 2 professional references within last 2 years

Additional Notes

• Non-expired photo ID required for submission
• Badge photo required (headshot, plain background, JPEG format)
• Background check must be updated if there is a break in assignment of 6 months or more
• Attestations are generally not accepted except for fingerprinting pending results with proper documentation
