Job Description
- Position at a medical/surgical unit within a hospital system in Wichita, KS.
- Long-term assignment expected to last approximately 13 weeks.
Required Credentials and Onboarding Requirements
Licenses & Certifications
- Nursys Verification: Must be completed within 30 days prior to start. Disciplinary actions require additional documentation.
- Current Nursing License: Primary source verification within 30 days prior to start.
- BLS (AHA): Required before starting.
- NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
- MOAB Moderate Risk De-Escalation Training: Required.
- Fit Test: Required for all travelers; preferred before start, can be done onsite. Annual update required.
- MMR: Proof of two vaccines or positive titer required; no declinations accepted.
- Varicella: Proof of immunity or two vaccines required; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
- Ascension 12 Panel Drug Screen: Within 30 days prior to start and annually thereafter.
Professional Documentation
- Resume/Profile: Required for submission.
- Two Professional References: Required prior to first assignment.
- Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or after breaks in service.
- Employment Verification: Past 7 years, updated annually or after breaks in service.
- Education Verification: Relevant clinical education verified within 90 days of start; updated after breaks in service.
- Driver’s License/State ID: Must be current and match address on file.
Onboarding & Testing
- Ascension Core Competencies: HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly.
- Ascension EKG Exam: Passing score of 80% required; two attempts allowed.
- Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
- Ascension Skills Checklist: Clinical skills checklist completed within one year of start; updated annually.
- Ascension Nursing Contract Worker Onboarding: Completed onsite.
- Ascension Onboarding Education Attestation: Completion of pre-hire education modules required.
- Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
Health & Safety
- Health Statement: Statement of good health or physical exam prior to first assignment; updated annually.
- Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Required if applicable; must be current.
- Hepatitis B: Documentation of vaccination series completion or signed declination.

- Some optional documents may be requested but are not mandatory for onboarding.
- All required documents must be current and compliant with hospital and regulatory standards.
- Compliance with all credentialing and onboarding requirements is mandatory prior to assignment start.
