Transfer Center - Clinical (RN/LVN)

Location: Rancho Cordova, California
Contract Length: 13 weeks

Position Overview

• Work in the Transfer Center facilitating patient transfers (any patient type) from hospitals within and outside the system.
• No hands-on patient care required.
• Manage approximately 12 cases per shift, often handling 10-15 cases simultaneously.
• Responsibilities include patient assessments, gathering clinical information, and coordinating with accepting physicians and specialists to secure appropriate bed placement.
• LVNs primarily focus on referring patients to external facilities if internal placement is unavailable.

Required Experience & Skills

• Minimum 2 years of experience in ICU, Emergency Department (ED), or transfer center.
• Acute care experience is mandatory.
• Critical care assessment skills and strong clinical judgment required.
• Ability to follow clinical algorithms.
• Professional demeanor with excellent customer service skills.
• Strong multitasking ability and proficiency with multiple computer applications.
• Experience with Cerner or EPIC electronic medical records (EMR) systems.
• Comfortable using TeleTracking for bed management and Tiger Connect for communication.

Certifications & Licensure

• Valid RN or LVN license required.
• BLS certification required.

Scheduling & Work Environment

• Must be available to work any day of the week.
• Every other weekend required.
• Must be available to work holidays as needed.
• No floating to other units.
• Orientation includes 1 day general hospital orientation plus 3-5 days department-specific training, which may occur on any shift.
• Dress code: Clean, professional scrubs of any color or casual attire (jeans without rips/tears, clean hoodies without large writing).

Additional Information

• Candidates cannot have been employed by any associated facilities within the past year, except PRN through outside firms.
• A dyslexia exam is required prior to acceptance, with two attempts allowed. Failure disqualifies candidates from this system.
• Limited return-to-office accommodations; remote work is rarely possible.
• Free onsite parking and EV charging available.
• Café closes at 2:30 pm.

• Communication: Tiger Connect, Google applications (including multiple chat platforms)

This role is ideal for healthcare professionals with acute care and transfer center experience who are comfortable managing multiple cases remotely and coordinating complex patient transfers.
