Job Description
- Physical Therapist position in Home Health setting covering Palmdale/Lancaster area.
- Work schedule options: Tuesday-Saturday or Wednesday-Sunday.
- Local candidates are welcome to apply.
- Home Health experience preferred.
- California license preferred; candidates with pending CA license who have applied may be considered.
- OASIS experience required; candidates without OASIS experience may be considered if they have strong Home Health experience.
- Guaranteed hours based on completed visits: 25 visits = 40 hours.
- COVID exemptions permitted.
- Mileage reimbursed at IRS standard rates.
Required Credentials & Certifications
- Licensure: California Physical Therapy license (pending applications accepted).
- Certifications:
- ACLS (Advanced Cardiac Life Support) – AHA Instructor or Provider acceptable, as applicable.
- BLS (Basic Life Support) – AHA Instructor or Provider required for all.
- PALS, NRP, EFM – as applicable (check credential requirements).
- National Tech Certification Verification – as applicable for allied professionals.
- State License Verification – primary source online verification required.
- NURSYS Verification – required for RN, LPN/LVN, AP (if applicable).
Required Documents & Compliance
- Government-issued photo ID (Driver’s License or State ID).
- Valid passport copy before start.
- Background checks including 7-year history, EPLS/GSA/SAM, HHS/OIG (updated annually).
- Photo for badge (passport style, shoulders up, front facing, plain background).
- Personal or auto insurance with current expiration dates.
- Auto registration with current expiration dates.
- Facility-specific training and policy acknowledgments (e.g., hand hygiene).
- Physical exam within 12 months of start.
- Immunizations and health screenings:
- TDAP vaccine within past 10 years (no declinations accepted).
- Negative TB test or chest X-ray within 1 year (with special protocols if past positive).
- Proof of immunity or vaccination for Measles, Mumps, Rubella (MMR) for candidates born in 1957 or later.
- Hepatitis B titer or vaccine series (declination accepted if within 1 year).
- Varicella immunity or vaccination (no declinations accepted).
- Negative 9-panel drug screen within 1 year.
- Seasonal flu vaccination/documentation (declinations accepted).
- COVID-19 vaccination or approved medical/religious exemption required prior to start, including booster dose.
Competency Requirements
- Medication Test/Pharmacology Competency Exam (as applicable) with minimum passing score of 80% or meet/exceed Angoff score.
- Competency Exam relevant to profession with minimum passing score of 80% or meet/exceed Angoff score.
- Annual skills checklist.
References
- Two professional references required.
