Physical Therapist - Home Health

Location: Oakland, California (Solano County Territory, with possible expansion)
Specialty: Rehabilitation Therapy
Employment Type: Contract
Contract Length: 13 weeks

Role Responsibilities

• Prioritize start of care visits
• Supervise Physical Therapy Assistants
• Mandatory next day reporting of start of care visits
• Clinical notes and documentation due within 48 hours of visit
• Maintain timely communication with supervisor, intake, and office
• Be generally responsive during normal business hours

Requirements

• Valid California Physical Therapist (PT) License
• Minimum 1 year of Home Health experience

Schedule

• 40 hours per week
• Workdays: Saturday through Wednesday

Training

• 3 full days (1 in-house, 2 in the field)

Reimbursements

• Mileage reimbursement for travel between first and last patient visit
• Parking and toll reimbursement

Productivity Expectations

• 30 points per week required
• Point to hourly conversion: 1 point = 1.33 hours
• Caseload and visit mix averages 8 hours per day, 40 hours per week
• Overtime requires pre-approval by Clinical Supervisor
