Description
Physical Therapist - Home Health
Location: Oakland, California (Solano County Territory, with possible expansion)
Specialty: Rehabilitation Therapy
Employment Type: Contract
Contract Length: 13 weeks
Role Responsibilities
- Prioritize start of care visits
- Supervise Physical Therapy Assistants
- Mandatory next day reporting of start of care visits
- Clinical notes and documentation due within 48 hours of visit
- Maintain timely communication with supervisor, intake, and office
- Be generally responsive during normal business hours
Requirements
- Valid California Physical Therapist (PT) License
- Minimum 1 year of Home Health experience
Schedule
- 40 hours per week
- Workdays: Saturday through Wednesday
Training
- 3 full days (1 in-house, 2 in the field)

Reimbursements
- Mileage reimbursement for travel between first and last patient visit
- Parking and toll reimbursement
Productivity Expectations
- 30 points per week required
- Point to hourly conversion: 1 point = 1.33 hours
- Caseload and visit mix averages 8 hours per day, 40 hours per week
- Overtime requires pre-approval by Clinical Supervisor
