Job Opportunity: RN Occupational Health

Location: Somerville, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Work Schedule

  • Option to work either:
    • 8 hours per day, 5 days a week, or
    • 10 hours per day, 4 days a week

Required Credentials & Documentation

Personal Health History Requirements (Must be completed prior to onboarding)

  • MMR Vaccine or Immunity Proof:
    Documentation of two measles, two mumps, and one rubella vaccine OR two MMR vaccines, or proof of immunity via blood test. Vaccine required if titer shows low or no immunity.
  • Influenza Vaccine:
    Required during flu season (Sept 15 - April 30). No exemptions allowed.
  • TB Test:
    Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date. Must be updated if there is a break in service longer than 6 months.
  • Health Statement:
    Physician statement signed by MD/DO/NP/PA within 365 days of start date; updated if >6 month break.
  • Respirator Medical Clearance (RMC) Attestation:
    Required if fit testing is needed for the role; must be current and updated if medical status changes.

Optional/Preferred Personal Health Credentials

  • Hepatitis B:
    Strongly preferred to have record of 3 vaccines or proof of immunity or declination.
  • Tdap Vaccine:
    Preferred within last 10 years and updated upon expiration.
  • Positive PPD - Negative Chest X-Ray:
    If TB test positive, chest x-ray documentation showing no active TB is required.
  • TB Symptom Survey (if Positive PPD):
    Recommended to complete prior to start and update annually.

Professional Requirements

Required for Submission

  • Resume:
    Include past 7 years of work experience with city and state for all positions and education.
  • References:
    Two recent references, at least one supervisor or higher in the relevant skillset.
  • Skills Checklist:
    Must be completed and submitted.
  • Candidate Submission Coversheet:
    Current version required for submission.

Required for Onboarding

  • Criminal Background Check:
    Must be completed within 45 days prior to start and updated if break >6 months. Includes county and international searches if applicable.
  • Massachusetts CORI Check:
    Completed within 30 days prior to start and updated if break >6 months.
  • Education Verification:
    Verified highest level of education achieved (not required for CNA).
  • SSN Trace:
    Completed with criminal check, listing addresses/aliases for past 7 years.
  • National Sex Offender Check:
    Completed within 30 days prior to start and updated annually.
  • Exclusion Checks:
    Completed within 30 days prior to start and annually; includes OIG, GSA, MassHealth, and NH Medicaid exclusion lists.

Additional Onboarding Documentation

  • Confirmation of Assignment/POI Form:
    Required for all initial assignments and extensions; must be updated if start date changes.
